We will provide you with a prescribed request form, "Request for Disclosure of Personal Information, etc.", which you should complete and return by mail to the address listed in "7. Contact Information", along with identification documents, agent verification documents, etc. Additionally, for notification of purpose of use and disclosure of personal information or third party provision records, a handling charge of 1,000 yen per request will be collected. Please transfer the fee to the account provided on the invoice. Please note that this fee applies even in cases where disclosure is not provided or purpose of use is not notified.
